Incoming Grade 7
- Original & 2 Photocopies of Report Card/School Form 9 back-to-back
- 3 Photocopies of PSA Birth Certificate
- 3 Copies of recent 2×2 ID picture for Student with nametag and white background
- 1 Copy of recent 2×2 ID picture for Parent/Legal Guardian with white background
- Latest Income Tax Return/Certificate of Tax Exemption from BIR
- ESC Application form and ESC Grantee Enrolment Contract (Will be given to the applicants once the requirements are completed)
Transferees (Grade 8-10)
- Original & 1 Photocopies of Report Card/School Form 9 back-to-back
- 2 Photocopies of PSA Birth Certificate
- 1 Copies of recent 2×2 ID picture for Student with nametag and white background
- 1 Copy of recent 2×2 ID picture for Parent/Legal Guardian with white background
- ESC Certificate (For Gr 8-10 who are ESC Grantees from previous schools)
Senior High School (Grade 11 & Grade 12 Transferees)
- Original & Photocopy of Report Card/School Form 9 back-to-back
- 2 Photocopy of Certificate of Junior High School Completion
- Certificate of Good Moral
- Photocopy of Diploma
- 2 Photocopy of PSA Birth Certificate
- 1 Copy of recent 2×2 ID picture for Student with nametag and white background
- ESC/QVR Certificate if applicable

Enrollment Procedures
Onsite Enrollment
- Proceed to the enrollment area to secure your schedule. Bring the required documents for enrollment, placed in a white long folder with proper label.
- Pay the required amount at the UB Cashier. PTA Fee (Php200.00) shall be collected by the PTA Officer / Enrollment Adviser
- Go back to the enrollment area for the printing of your schedule.
- Process your ID and GMAIL ACCOUNT at the MIS Office.
